Provides comprehensive child’s well visit to income eligible children, infancy to 18 years of age,
by offering physical examination, laboratory testing, vision and hearing testing, developmental
testing and immunizations.

Care Coordination for Children (CC4C)

(Medicaid & other eligible children)
CC4C is a care management program for children birth to 5 years of age. Children are referred that have, or are at risk for, chronic physical, developmental, behavioral, or emotional conditions that may need health or related services for an extended amount of time. Children are also accepted into the program who do not have a medical home or children who are exposed to toxic stress. Referrals are made for well visits, immunizations, and/or developmental screenings. Information on local resources and programs are offered to families as needed.

The Alexander County Health Department follows the recommended guidelines for immunizations that the ACIP (Advisory Committee of Immunization Practices) and the CDC (Centers for Disease Control and Prevention) and the NC Immunization Laws & Rules.

  • Alexander County Health Department offers vaccinations for infants and children through age 18.
  • Immunizations required for routine infant vaccination, daycare or school attendance or work required are available
  • Hepatitis B
  • DTaP (Diptheria, Tetanus and Pertussis)
  • Hib (Haemophilus influenza type b)
  • IPV (Polio)
  • PCV7 (Prevnar or Streptococcus pneumonia)
  • MMR (Measles, Mumps, and Rubella)
  • Varicella (Chickenpox)
  • Influenza (Flu)

Cost: Routine childhood vaccinations are paid for by federal and state funds if certain criteria is met. This is to encourage parents to protect their children against vaccine-preventable diseases

Criteria for Free vaccine:

  • Age 0-18
  • Medicaid
  • No insurance
  • If your insurance does not cover cost of vaccines

Walk-in (no appointment needed)
Tuesday 1pm-4:30pm
Wednesday 1pm-4:30pm
Friday 8:30am-12 noon

Child Health Clinic:
No walk-in (by appointment only)
Monday–Friday 8am-5:00pm
